You may enter limited personal details such as your name, procedure date and time, and driver or contact information to power reminders and summaries. You choose what to enter.

Providers and clinics
If you access MyPrepGuide through a clinic or QR link, branding you see may reflect that relationship. Clinic accounts are governed by separate agreements between the clinic and MyPrepGuide.
